Patient-Centered Medical Home
Barochia Internal Medicine is proud to be a Patient-Centered Medical Home (PCMH). To achieve this goal, we have fully integrated electronic health records (100% paperless), which interfaces with The Hospital of Central Connecticut , Quest Diagnostic and LabCorp. Some of its features include a patient portal, electronic prescriptions, and auto appointment reminders. The patient portal allows patients to access their health records, view upcoming appointments, and email the doctor with any questions or refill requests.
Dr. Barochia coordinates every aspect of your care in the office and at the hospital. This way, you get better attention and proper care and nothing gets overlooked or unnecessarily repeated.
When you choose a PCMH, you are choosing a primary care provider who you will call every time you need medical attention. If you see the same doctor every time, he will know your medical history and which medications you are taking, and you will develop a relationship with your doctor and his healthcare team.
As your doctor or provider of choice, Dr. Barochia will:
- Work with you to improve your health and address your questions to help you make informed decisions about your care
- Explain diseases, treatments, and test results in an easily understandable manner
- Review your medications at every visit and discuss with you any interactions or contraindications
- Provide timely access to medical care and same-day appointments, whenever possible
- Provide the care you need when needed
- Develop a personal action plan with you to address your chronic conditions
- Help you set goals and track your progress
- Provide you 24-hour access to a clinical decision maker by phone
- Send you to trusted experts, if needed
- End every visit with clear instructions about expectations, treatment goals, and future plans

We trust you, our patient, to:
- Be honest about your medical history, symptoms, and other important information about your health
- Tell your doctor about all medications you are taking
- Ask questions and actively participate with your doctor in planning your care
- Adhere to your doctor’s recommended treatment plan
- Inform your doctor of any changes in your health and well-being
- Let us know when you see other doctors and what medications they may put you on or change
- Ask other doctors to send us a report about your care when you see them, including lab work, test results, X-rays, or other diagnostic imaging tests
- Prepare for your appointment by getting your lab work done before the day of your appointment so your doctor can go over the results with you during the appointment time
- Keep your appointments as scheduled whenever possible
- Know your insurance and what it covers
- End every office visit with a clear understanding of your doctor’s expectations, treatment goals, and future plans for you
